Care Management 
Care management is an important service that includes coordinating care and services that will help you deliver on the goals you identified in your Care Plan. Hours of service are estimates only and relate to the time it currently takes to cover the three key case management tasks: coordination and scheduling of services, budget management, and creating the task schedules. We will come to visit you for your reassessment at regular intervals to deliver a better service. In particular, if there is a change of situation, we are required to conduct a reassessment to provide your assistance.

Package Management 
Package management is the ongoing administration and organisational activities associated with ensuring the smooth delivery and management of your Home Care Package. It may include the costs for: preparing monthly statements; managing your package funds; and compliance and quality assurance activities required for Home Care Packages.

Home Based Support Services

In home respite care; Personal Care; Domestic Care (housework, meal preparation medication monitoring, shopping); Community Access; Social Support

Other Costs

Other costs relate to other prices the provider may charge you out of your total package amount.

Notes on the Delivery of Services

Cancellations: A minimum of 24 hours notice of cancellation is required otherwise the full rate is charged. This charge may be waived in the event of an emergency 

Sleepovers: In the case of sleepovers and rostered overnight care, in order for the worker to sleep or live in the designated place of work we must include the provision of suitable accommodation and access to a telephone
